Randy sent me the photo of the white daisies the other day. Several years ago I had a large sunflower that folded up like that, one big mutant in the whole seed packet....makes me wonder where the seeds came from.
It's called fasciation and there can be many causes, hormonal, fungi, bacteria, genetics or environmental. If it's a perennial it might very well not be there the next year it grows but if it is a genetic cause than most likely will. I've seen a few of these many years ago in my grandmother's garden.
I have a Shasta daisy in my garden like that. It is not as pretty as the one shown or as perfectly shaped. But it is definitely double. Here are a couple of pictures.
